Sponsorship Request Form
The 5 Star Lady Mafia travel softball team is currently forming its Spring 2023 travel softball program for the youth in our community.
We want to extend an invitation to you and/or your company to sponsor our softball team.
This can be your chance to be part of the fun for these young girls and be recognized by the community as leaders in youth sports.

This season we will be offering (4) different levels for our sponsors. Each will be designed around exciting and extensive packages, driven towards getting your business and name in the public market.

1) SINGLE: $150 & UP The "Single" Sponsorship will receive a team photo in a beautiful plaque.
2) DOUBLE: $250 & UP The "Double" Sponsorship will get their logo on our team's website, along with a beautiful team photo plaque.
3) TRIPLE: $500 & UP The "Triple" Sponsorship will receive their company name/logo on our team website, name or logo printed on our elite team BANNER, and a beautiful team photo plaque.
4) HOMERUN: $1000 & UP The "Homerun" Sponsorship will receive their logo as a sponsor on the girl's warm-ups, their company logo on our team website, name/logo printed on our elite team banner, Social media mentions on the game days and a beautiful team photo plaque.
